Wide complex tachycardia
#ECG of a hypotensive 75-year-old man who presented with syncope
1. Wide-complex, regular tachycardia that looks like this is V-Tach until proven otherwise
2. Remember that the most effective “antiarrhythmic” when your patient is unstable is a sedative + electricity!#FOAMed⚡ pic.twitter.com/8DflwkMTEz— Sam Ghali, M.D. (@EM_RESUS) January 20, 2018
“Slow” wide complex tachycardia
#ECG of an unconscious young man thought to be in "V-Tach"
His rate is ~110 bpm
When the rate is <120, think #Hyperkalemia
K+ was 9.5!If you can recognize this without blood work one day you will save a life!#FOAMed pic.twitter.com/hPjPMBJslf

Hyperkalemia
Cath lab activated for this #ECG. I told ER to cancel STEMI and check potassium. K=9.5 and Cr 7.2. Needs dialysis STAT! #SineWaves #FOAMed pic.twitter.com/JDByipWo9m

Inferior STEMI
40-year-old lady with chest pain x 12 hrs
This is what Subacute STEMI looks like on #ECG. II, III, & aVF have well-developed Q waves and T waves have become biphasic. If the patient still has symptoms emergent PCI is indicated. Cath showed 100% proximal RCA Occlusion!#FOAMed pic.twitter.com/vsP1fWXE78

The key to #ECG reading is to recognize patterns. But the computer doesn't recognize patterns, it just measures millimeters. There is ST Elevation with big, round, symmetric T-waves in II, III, & aVF. There is reciprocal T-wave inversion in aVL. RCA was 100% occluded!#FOAMed pic.twitter.com/UHbq6iym8J

Cardiac tamponade
This is a good one. Pt with SOB, hypotension, in distress. Diagnosis is in the ECG. pic.twitter.com/rQC8A61qgt
